The completion of this form will provide our trainers with issue-specific information that members of the media may ask you about in relation to a news story, feature article or backgrounder. We ask that you choose TWO real issues, situations or concerns that you and/or your organization face — ones that may be difficult to deal with in terms of the media.

PLEASE ENSURE THAT WE RECEIVE YOUR COMPLETED QUESTIONNAIRE WITH ANY RELEVANT BACKGROUND INFORMATION (e.g., press clippings, video/audiotapes, news releases, communications plans, etc.) AT LEAST ONE WEEK PRIOR TO YOUR SCHEDULED TRAINING.
